Please Provide Your Personal Info for Tax and Background Check Purposes
The purpose of the W-9 form below is to collect your name, address, and social security number so that MyGuru can both:
a) issue you a 1099 form for tax purposes at the end of the year, telling the IRS how much money you've earned through MyGuru. As an independent contractor, no taxes will be taken out of your paychecks, and you'll use this 1099 to calculate your own taxes.
b) run a background check on you, to provide our students/customers with assurance that you are not only an experienced, impressive tutor, but free of any criminal record.
Regarding this background check, the W-9 form provides almost all of the required info we need to run this background check, except for your date of birth. To provide your date of birth, please remember to fill out the extremely short form below the W-9 agreement.
If anything shows up on your background check, we will notify you.
